Established back on December 29th, 1970, the Occupational Safety and Health Administration (OSHA) became established under the Occupational Safety and Health Act. It's a division of the U.S. Department of Labor and works to make sure that there are safe and healthy working conditions for employees throughout the United States. It also provides education, outreach, training, and assistance for workers throughout the country. When companies are deemed to have had violations at their places of operation, they can result in fines from OSHA.
This infographic takes a look at the highest OSHA fines by State, dating back to 2015. The highest in the United States since that time occurred in Hebron, Ohio in 2015. Sunfield Inc. was found to have 46 violations which combined, resulted in a fine of $3,420,000. One of the workers had suffered severe lacerations (even after the machine malfunction was noted by a supervisor) and needed an arm amputation.
